JAPAN VISA REQUIREMENTS

1. ​PASSPORT

• Validity of more than 6 months & Signed Passport
• Broken lamination of the Picture Portion will not be accepted / Must have at least 2 blank pages left in the passport.

2. VISA APPLICATION FORM - TYPEWRITTEN ※（A facial Photo (4.5×3.5cm) must be attached.）

    PHOTO (2 Copies of 4.5cm x 3.5cm with white background.)

• Specs: 4.5cm x 3.5cm, with white background.
• Photo must be taken within 6 months.
• Please write applicant’s name and birthdate on back side of the photo.
• Photo must be pasted on the application form.​
• No smiling. Photo showing teeth is not allowed. No earrings or any accessories.

(3) PSA issued Birth Certificate and Marriage Certificate (for married applicants), issued within 1 year
☞ Unnecessary if there is used Japan Visa on passport.

【ADDITIONAL REQUIREMENTS】
- If (3) is unreadable, submit Birth/Marriage certificate issued by Local Civil Registrar.
- If Birth Certificate is “LATE REGISTRATION”, submit Baptismal Certificate and School Record (Form 137). (Need contact tel # of the church,and contact tel# of the school)
-If there is no record of Birth/Marriage in PSA, submit Birth Certificate issued by Local Civil Registrar and Negative Certificate issued by PSA. 

(4) TOUR ITINERARY 

• Prepared by the applicant or the travel agency.

【In case that applicant will shoulder part/all of travel expense】

(5) Applicant’s Bank Certificate (balance within the last six months must be shown) * If the Average Daily Balance (ADB) in the last six months is not indicated, bank statement must be submitted to prove transactions within the last six months.

(6) Applicant’s Tax Payment Certificate * Form 2316, must have signature of the Employer and Employee * For business owners, proof of actual payment of tax must be submitted in addition to the BIR Tax Payment Certificate Form (copy acceptable)

(7) Applicant’s Employment Certificate (must indicate period of employment, salary and position) * For business owners, Department of Trade and Industry (DTI) “Certificate of Business Name Registration” and Mayor’s Permit from the City Hall must be submitted.

* If for any reason other than being a retiree, full-time housewife or unemployed, applicant is unable to submit (6) and (7) above, a letter of explanation about the reason must be submitted.

For students, a copy of School ID or Certificate of Enrollment in School must be submitted.

• Size of document for application should be A4 size only.
• If document is other size, please submit copy that is already adjusted in A4 size from its original and without staples or pasting pages.
• Applicants are exempted from submitting (4) and (5) Indicated above if they can submit old/valid passports with used Japan visa.
• It is applicant’s responsibility to ensure that he/she meet the requirements for the grant of a visa.
• Submission of any supporting documents not listed above is encouraged (e.g. applicant’s economic or social ties with the Philippines, urgent reasons for visit: medical certificate, wedding invitation).

HOW TO FILL OUT THE JAPAN VISA APPLICATION FORM FOR TOURISTS

Before anything else, take note of these general guidelines:

• Print the application form on an A4 paper only. Other sizes are not accepted.
• Fill out the form either digitally or by hand. Both computerized (typewritten) and handwritten are accepted. But for handwritten forms, make sure that you write in block letters and that they are readable.
• Use a black pen. Do not use erasable pens or pencils! Likewise, if you fill it out digitally, use black font.
• Don’t use correction tape or liquid. Try to avoid any erasure. If something needs to be corrected, use double line (2 strikethroughs). If there are too many erasures, best to start all over with a fresh blank form.
• Don’t leave any field blank. If the item or question doesn’t apply to you, write NA or N/A, which is short for Not Applicable.
• Do not staple the form! The agency will most likely remove them too.
