​REQUIREMENTS:
  1. ​PASSPORT
  • Validity of more than 6 months / Signed Passport
  • Broken lamination of the Picture Portion will not be accepted / Must have at least 2 blank pages left in the passport.
2. VISA APPLICATION FORM - TYPEWRITTEN

3. PHOTO (3 Copies of 4.5cm x 3.5cm with white background.)
  • Specs: 4.5cm x 3.5cm, with white background.
  • Photo must be taken within 6 months.
  • Please write applicant’s name and birthdate on back side of the photo.
  • Photo must be pasted on the application form.​
  • No smiling. Photo showing teeth is not allowed. 
4. BIRTH CERTIFICATE
(PSA issued with receipt and valid within (1)year.)
(Submission of PSA certificate is waived with submission of OLD USED JAPAN VISA.) Applicants holding (old or valid) passports with used Japan Visa will be exempted from submitting of birth certificate.
  • IT must be issued within one year from PSA Main Office/Serbilis Outlet Center (Nationwide).
  • If the birth certificate from Philippine Statistics Office (PSA) is unreadable, or has incomplete information, please submit the birth certificate issued by PSA together with a birth certificate issued by the Local Civil Registrar.
  • If the birth certificate is “late registration”, please submit in addition, a baptismal certificate and school record (Form 137) from high school or elementary and School Yearbook (if possible).
  • If there is no record in PSA, please submit a “Certificate of Non-Record” from PSA together with a birth certificate from the Local Civil Registrar.

5. MARRIAGE CONTRACT (If Married)
PSA issued and valid within (1)year.
(Submission of PSA certificate is wived with submission of OLD USED JAPAN VISA.) Applicants holding (old or valid) passports with used Japan Visa will be exempted from submitting of marriage contract.
  • In case of non-record.You must submit certificate of non-record together with the one from local civil registrar.
6. TOUR ITINERARY 
  • Prepared by the applicant or the travel agency.
7. BANK BALANCE CERTIFICATE
  • Validity of Bank Certificate is three (3) month from the date of issue
  • We strongly suggest to have at least 100,000 in your bank account. If you are sponsoring, of course higher than that amount.
8. LATEST INCOME TAX RETURN (Form 2316 or 1701) Original and (1) photocopy. 
Prepare Letter of Explanation of No ITR if you do not have ITR. 
9. CERTIFICATE OF EMPLOYMENT
10. VISA PROCESSING AUTHORIZATION LETTER.

  • Size of document for application should be A4 size only.
  • If document is other size, please submit copy that is already adjusted in A4 size from its original and without staples or pasting pages.
  • Applicants are exempted from submitting (4) and (5) Indicated above if they can submit old/valid passports with used Japan visa.
  • It is applicant’s responsibility to ensure that he/she meet the requirements for the grant of a visa.
  • Submission of any supporting documents not listed above is encouraged (e.g. applicant’s economic or social ties with the Philippines, urgent reasons for visit: medical certificate, wedding invitation).

HOW TO FILL OUT THE JAPAN VISA APPLICATION FORM FOR TOURISTS

Before anything else, take note of these general guidelines:
  • Print the application form on an A4 paper only. Other sizes are not accepted.
  • Fill out the form either digitally or by hand. Both computerized (typewritten) and handwritten are accepted. But for handwritten forms, make sure that you write in block letters and that they are readable.
  • Use a black pen. Do not use erasable pens or pencils! Likewise, if you fill it out digitally, use black font.
  • Don’t use correction tape or liquid. Try to avoid any erasure. If something needs to be corrected, use double line (2 strikethroughs). If there are too many erasures, best to start all over with a fresh blank form.
  • Don’t leave any field blank. If the item or question doesn’t apply to you, write NA or N/A, which is short for Not Applicable.
  • Do not staple the form! The agency will most likely remove them too.
